To assist in the nomination and assessment process, please provide as much information about the tree(s) and be as specific in your descriptions as possible. Where necessary, information will be verified by staff from the City of Melbourne and a qualified arborist.


Essential information

Please complete all components in this section

Please provide the most precise address of the nominated tree/trees, for example, ‘South eastern corner of rear yard at 12 Oak St. Melbourne’. If property address is unknown, or if the tree is not on a discernable block, then the address of the adjacent or facing property should be provided.


Please attach a photograph/ photographs of the nominated tree(s), showing the tree(s) in full, as well as details of the bark, leaves, fruit etc.

You may also attach any supporting documentation which may help to illustrate the exceptionality of the tree(s), for example, local history information linked to the tree(s), photographs, or evidence of nomination support from other members of the street/local community.
