The Home and Community Care program funds a range of services designed to support you or the person you care for, to stay active and independent and living at home for as long as possible.
Helping people stay active, involved and doing as much for themselves as possible is proven to help them stay more independent and have a greater sense of wellbeing. This is the approach we take when working with you.
Services can include:
- Around the house: working with you to keep your house clean and safe and helping you with basic maintenance tasks inside and out.
- Eating well: helping you to eat well by assisting with shopping; working beside you to prepare meals or providing delivered meals.
- Out and about: connecting you with an interest group or social opportunities; helping you get to places you want to go.
- Self-care: working with you to manage personal tasks such as having a shower and getting dressed.
- Managing your health: access to a range of professional services such as nursing, physiotherapy, occupational therapy and podiatry in your home or at a centre, helping you to manage your health issues and build your strength and ability to stay active.
We work with you and the people who support you to find out the things that you want to achieve and the things that are important to you. We then help to put in place the right solutions for you to help maintain your independence. Fees are charged according to income.
Living at home assessment
Once eligibility is determined and a referral has been accepted, an intake officer arranges an appointment for an assessment officer to visit the client at their home to undertake a Living At Home Assessment. This assessment involves discussing the client’s current strengths, capacity, resources and needs, in addition to identifying any suitable services that may support the client to remain active and independent.
Reviews are conducted to ensure that services continue to meet each client’s needs. Clients can contact their assessment officer or case manager at any time to discuss a change in their situation.
Case management
The City of Melbourne operates two programs for residents who require a higher level of support and coordination of services:
- Community aged care packages program (available to people aged over 65)
- Linkages program (available to people of any age with a disability)
Both programs assist people to remain at home who may otherwise enter residential care. Clients are assisted by a case manager who helps to coordinate and plan appropriate services.
